A Mrtyu Praman Patra is a very important document. It certifies the death of any person. Which is issued to the next of kin of the deceased person, It contains the complete facts of the death and the reason for the death.

To provide proof of time and date of death, to free the person from social, judicial, and governmental obligations, to dispose of property-related heritage disputes to authenticate the fact of death, and to deposit insurance and other benefits to the family.

It is very mandatory to register the death to authorize. The Mrtyu Praman Patra is officially issued by the Urban Development Department of the state. Under the law in India (as per the Birth and Death Registration Act, 1969) it is mandatory for every death to be registered within 21 days of its occurrence in the concerned State / Union Territory.

Accordingly, the Government has provided a well-defined system for registering deaths in the registrar premises in the villages and registrars in the villages with the Registrar General at the Center, near India, and with the Chief Registrars in the States.

Why Need To Getting Death Certificate

The report of the “Mrtyu Praman Patra” of friends or its registration is done by the head of the family, by the medical in-charge if the person dies at home, or in the hospital, if the death is in jail, by the jail in charge if If the dead body is lying, registration is done by the head of the village or the local place in-charge. To create Mrtyu Praman Ptra, you must first register the death.

The registration of the death has to be done with the concerned authority within 21 days of the death by filling the prescribed form by the registrar, after which the Mrtyu Praman Patra is issued after proper verification. If death is not registered within 21 days of a death, permission is required from the Registrar / Area Magistrate along with the prescribed fee and if there is delayed registration.

The application form in which you need to apply is generally available with the local body authorities or registrars of the area who maintain the death register. You may also be required to submit proof of birth of the deceased person, a promissory note specifying the time and date of death, a copy of the ration card, and the requisite fee in the form of a court stamp.

Documents Required for Death Certificate

The following documents are required to make a death certificate in Uttar Pradesh. Which are as follows.

  • An identity card such as Aadhaar card, voter identity card, Aadhaar card, PAN card, ration card,  Death Proof by Medical Officer / Village Prathan / Vidayak or Sansad / MBBS Doctor / City Counselor.
